Our hospice Team has over 30 years of experience in Hospice and Long Term Care. We are dedicated and compassionate in our determination to advocate for quality end of life care, comfort and support for our patients and loved ones.
Our Belief
We believe that every person has the right to live life free of pain and to die with dignity. Horizon Hospice affirms life, and neither hastens nor postpones death, but exists in the hope and belief that through comfort care that is sensitive to the client’s own specific needs, that they and their caregivers will be better prepared to meet the challenges in end-of-life.
We believe that your quality of life is just as important as the length of your life. We recognize dying as a part of the process of living and are focused on elevating the quality of your end-of-life journey.
Hospice Is
An intensive program that provides special care for anyone who might have a diagnosis or decline in health that could limit their life expectancy to 6 months of less if the disease/decline runs its normal course. This is determined by your Primary Care Physician based on established end-of-life guidelines.
Hospice care concentrates on relief of any uncomfortable symptoms and pain, focusing on the individual, their quality of life statement, and their family and/or caregivers with an added emphasis on emotional and/or spiritual issues, as needed.
