Who Pays for Hospice Care?
Medicare has been reimbursing 100% for Hospice care for over 20 years. Medicaid, VA, and most private insurance companies have Hospice Benefits. We receive a per day or per diem rate to cover the cost of your Hospice care. We will be glad to verify your Hospice Coverage prior to any initiation of reimbursable services.
What Services are Provided by Horizon Hospice?
Our Medical Director is the leader of our clinical team. We provide a professional and caring Clinical Staff to do intermittent visits as determined by you, your Doctor, and our team to assess your needs and teach and or provide care as appropriate within hospice guidelines.
- Registered Nurse
- Home Health Aide and/or Homemaker
- Social Services and Spiritual Support
- Therapies & Dietary services, if needed
- Volunteers
On call 24/7 – One of our RNs is on call 24 hours a day, 7 days per week to answer any questions you or your caregivers have and make extra visits as needed.
Bereavement Services – We follow our families with grief support, as they wish, for 13 months after the loss of a loved one.
Medication and Medical Equipment and supplies – We provide and pay for any medication and or equipment / supplies needed related to your end-of-life diagnosis that is determined necessary by your Dr. and our team, for your comfort.

Who qualifies for Hospice Care?
Anyone who is dealing with a life limiting illness.
Anyone whose Doctor feels that the illness might have a life expectancy of 6 months of less if the illness continuous on is “normal course.” We would be glad to work with you and your Doctor to determine if your illness meets Hospice Criteria before moving forward with any care.
Anyone who has an illness that the Doctor says there’s no further curative treatment available or you no longer want to pursue active curative treatment for whatever reason.
Anyone who wants comfort measures only. Statistics show that early admission to hospice greatly improves quality of life for the patient and caregivers because the patient receives regular pain and symptom management leading to overall consistent comfort.
Statistics also show that approximately 10% of residents receiving care in a nursing home may qualify for Hospice Care.
Where is Hospice Care Provided?
Anywhere the patient calls “home”. Their private home or the home of a friend or family member, senior living, assisted living apartments, and nursing homes.
